TattooMe ACH Recurring Payment Authorization Form

By signing this form, you authorize TattooMe, located at 623 W Pacific Coast Highway #H, Long Beach, CA 90806, to initiate recurring ACH debit entries to the bank account indicated below for payment of your TattooMe software subscription.

Billing Authorization

You authorize TattooMe to charge your checking or savings account on a recurring monthly basis for your selected TattooMe subscription plan at the then-current monthly rate associated with your selected tier. Charges will be initiated on either the 1st or the 15th of each month, depending on your enrollment date. If your subscription includes a promotional or trial period, your initial debit will occur after the trial period ends, followed by recurring monthly debits.

Change in Amount or Date

If the amount or scheduled debit date changes, you will receive notice at least 10 days prior to the next scheduled debit.

Cancellation / Revocation of Authorization

This authorization will remain in effect until you cancel it. You may revoke this authorization by providing written notice at least 10 days prior to the next scheduled debit. Cancellation requests must be submitted by email to info@tattoome.net or by mailing written notice to: TattooMe 623 W Pacific Coast Highway #H Long Beach, CA 90806

Authorization Acknowledgment

I understand that this authorization is provided in electronic form and that I have the ability to print or save a copy of this authorization for my records. I acknowledge that I am authorizing recurring ACH debits and that these debits will appear on my bank statement as “ACH Debit – TattooMe.”
